Kidney (Renal) Cyst

Bosniak Classification

The Bosniak kidney cyst classification system evaluates renal cysts found on ultrasound or CT scans, categorizing them into five types (I to IV) based on imaging characteristics.

  • Type I (1): Simple, benign cysts with thin walls and no internal echoes; no follow-up needed.

  • Type II (2): Minimally complex cysts with thin walls and minor septa; generally benign but may require follow-up to monitor.

  • Type IIF (3): More complex than II, with thicker walls or multiple septa; follow-up imaging is recommended.

  • Category III (4): Cysts with thickened walls or irregularities that may indicate malignancy; further evaluation, including biopsy, is necessary.

  • Category IV (5): Definitively malignant cysts requiring immediate surgical intervention and further assessment.

The Bosniak classification helps healthcare providers determine management and follow-up for renal cysts. Regular monitoring is essential for higher-category cysts to address any changes promptly. Discussing the classification and next steps with your healthcare provider is important for effective management.
